We will attempt to contract with other smaller plans, however, unfortunately at this time Humana will not open their panel to us.

Please bring your insurance ID card to every visit. We also require that you verify your address (it needs to be the same as the one your insurance uses). If you do not bring your card or update your address that is your choice, but rather than spending hours tracking down the correct numbers, you will be held responsible for the full amount.

Cash Pricing

If you are not convinced of the need for insurance and have opted for a healthshare of some sort, then we can still see you for cash and you can request a statement to submit for reimbursement. Please call the office for current cash pay pricing.


These medications are classified as experimental and your insurance will NOT pay for them. Your insurance company may have problems with you using your health savings account for these medications – please verify that it is ok to use your HSA prior to your appointment.


Since the Affordable Care Act insurance plans have been required to cover an annual ‘physical’, the insurance companies view this service to be a check box list of preventive tests and screenings. If a preventive physical uncovers a problem (your blood pressure is elevated, depression, an ingrown toenail) and I have to do anything outside of the check list (prescribe a medication, send a referral, do extra tests) then the visit is no longer just a preventive physical and I have to charge an office visit in addition.

We do not charge the copay upfront for your physical because you may not need to pay a copay, but if it turns into an office visit you will be responsible for any copay or deductible as per your insurance.
